Conwy Connect for Learning Disabilities

	Person Centred Planning Service
	Conwy Connect for Learning Disabilities received funding from the “Big Lottery Fund” for a Person Centred Planning Project to run from April 2004 to April 2007. In that time people were given the opportunity to be trained in putting Person Centred Plans together. These people, called 'Facilitators', now form a group called the Foundation Team.
	Person Centred Planning can help people to: Plan for changes in their lives and make them happen. Make friends and relationships. Feel good about themselves. Get the right support. Take part in activities in the community. To put you at the centre of your plan.
	Who can benefit from our service? People with learning disabilities, who live in the county of Conwy, aged between 18-65. Parents/carers of a person with learning disabilities. People who support or work with people with learning disabilities in the county of Conwy.
