Familiar Face – New Role!!

I’m likely to be a familiar face to many of you, as I’ve been involved with Conwy Connect in various ways since 2005, but I’m here to tell you about an exciting new Service that Conwy Connect is about to launch.
We are grateful to the Trustees of the Lloyds Bank Foundation for agreeing to make a three year grant to fund the Planning for the Future Service. (And well done to Sue on making the successful application!!)
The aim of the Service is, in the words of the Lloyds Bank Foundation, to: Support individuals with learning disabilities and their parents/carers to plan for and help them during their transition into independent living.

As part of meeting the objectives set out in the grant application, we will be:
Identifying relevant families on Social Services’ accommodation list who will be moving out of the family home.
Planning directly with individuals and their families for a future move from the family home.
Carrying out two 6 monthly reviews for those who have had a plan and moved from the family home.
News of the grant was received in May, and work behind the scenes began in June, and to some extent has carried on behind the scenes.
The Planning for the Future Service was mentioned briefly in July at the Key to My Future event (do take a look at the article about this event), but is likely to be officially rolled out as part of the day for parents and carers “Keep Planning Together” on 8th September.
